Addressing Transportation Insecurity: Concrete Advice From Clinic Leaders

The Center for Care Innovations (CCI) partnered with Cedars-Sinai to support ten clinics in Los Angeles to build the capabilities needed to assess and address Social Determinants of Heath (SDOH) in a program called Moving Clinics Upstream (MCU). Social determinants have a major impact on health outcomes—especially for the most vulnerable populations. Factors such as a patient’s education, income level and environment must be considered when providing treatment and care.
